Case Manager - benefits navigation (Oakland, CA)

Rivermark Rowan Health provides primary and specialty care in Oakland, and we need a Case Manager to lead our benefits navigation. Continuing education is supported. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education. Working arrangement • Full-time, Monday to Friday. • On site in Oakland. • Applications are reviewed on a rolling basis.
