Case Manager - benefits navigation (Minneapolis, MN)

Clearpath Cobalt Property Management is looking for a Case Manager. The job centres on benefits navigation for unit turnovers, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for unit turnovers. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with unit turnovers.
