Case Manager - benefits navigation (Minneapolis, MN)

We are growing our team at Highbridge Sparrow Civic Services Group and adding a Case Manager whose main responsibility will be benefits navigation. Benefits and stability attract long tenure. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for constituent requests. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with constituent requests. • Bilingual English and Spanish. Working arrangement • Contract for twelve months, with clear deliverables from the first week. • On site in Minneapolis. • Applications are reviewed on a rolling basis.
