Case Manager - benefits navigation (Louisville, KY)

Northwind Ember Housing manages affordable housing communities across Louisville, and we need a Case Manager to lead our benefits navigation. Collaboration with local agencies is constant. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for maintenance stores. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with maintenance stores. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Louisville. • Applications are reviewed on a rolling basis.
