Case Manager - benefits navigation (Louisville, KY)

We are growing our team at Silverline Vista Community Partners and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people based in Louisville. What you will do • Connect clients with partner agencies. • Help with applications and appointments for client intake. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with client intake. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Louisville. • We reply to every applicant.
