Foxglove Cedar Health is looking for a Case Manager. The job centres on benefits navigation for referrals, and it would suit someone who likes steady, practical work. You would join a large organisation of about 367 people based in Louisville. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Help with applications and appointments for referrals. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with referrals. Working arrangement • Part-time, about 24 hours a week, afternoons. • On site in Louisville, with some flexibility after the first few months. • We reply to every applicant.
