Case Manager - benefits navigation (Little Rock, AR)

Westbrook Glen Care Services Group is looking for a Case Manager. The job centres on benefits navigation for care plans, and it would suit someone who likes steady, practical work. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for care plans. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 16 hours a week, mornings. • On site in Little Rock, with some flexibility after the first few months. • We reply to every applicant.
