Case Manager - benefits navigation (Kansas City, MO)

Greenridge Clear Care Services Group is a social-care provider serving older adults and people with disabilities around Kansas City. We are hiring a Case Manager to take on benefits navigation. Teams support each other, especially on hard days. What you will do • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for care plans. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with care plans. Working arrangement • Part-time, about 28 hours a week, spread across three days. • On site in Kansas City, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
