Case Manager - benefits navigation (Jacksonville, FL)

Ironwood Linden Health is a healthcare provider in Jacksonville with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Help with applications and appointments for insurance authorisations. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Contract for eight months, with a review at the end and a chance of extension. • On site in Jacksonville. • We will tell you what to expect at each step of the hiring process.
