Case Manager - benefits navigation (Jacksonville, FL)

Rivermark Lakeside Care Services provides in-home and community support in Jacksonville, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with licensing. Working arrangement • Part-time, about 24 hours a week, spread across three days. • On site in Jacksonville, with some flexibility after the first few months. • We reply to every applicant.
