Silverline Cobalt Housing Group provides affordable homes and support services around Jacksonville. We are hiring a Case Manager to take on benefits navigation. Collaboration with local agencies is constant. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for resident services. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Experience with resident services. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Jacksonville. • We reply to every applicant.
