Case Manager - benefits navigation (Jacksonville, FL)

We are growing our team at Westbrook Stone Care Services and adding a Case Manager whose main responsibility will be benefits navigation. Teams support each other, especially on hard days. What you will do • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with care plans. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Jacksonville. • Applications are reviewed on a rolling basis.
