Case Manager - benefits navigation (Jacksonville, FL)

Stonegate Nest Community Partners is a nonprofit in Jacksonville supporting families with food, housing and job help, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 45 people based in Jacksonville. What you will do • Help with applications and appointments for grant reporting. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with grant reporting. Working arrangement • Contract for six months, with the possibility of becoming permanent. • On site in Jacksonville. • Applications are reviewed on a rolling basis.
