Case Manager - benefits navigation (Houston, TX)

Greenridge Juniper Health Group is a healthcare provider in Houston with clinics and support teams. We are hiring a Case Manager to take on benefits navigation. Clinicians and administrators work side by side. What you will do • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Houston, with some flexibility after the first few months. • We reply to every applicant.
