Case Manager - benefits navigation (Honolulu, HI)

Bright Fern Care Services is looking for a Case Manager. The job centres on benefits navigation for family communication, and it would suit someone who likes steady, practical work. You would work closely with colleagues in operations, support and management. What you will do • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 16 hours a week, afternoons. • On site in Honolulu. • Applications are reviewed on a rolling basis.
