Case Manager - benefits navigation (Hartford, CT)

We are growing our team at Highbridge Meridian Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of more than 1,000 people based in Hartford. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for licensing. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with licensing. • Bilingual English and Spanish. Working arrangement • Full-time, with a steady weekday schedule. • On site in Hartford. • We reply to every applicant.
