Case Manager - benefits navigation (Hartford, CT)

Bright Clear Care Services Group supports families and individuals in Hartford with personal care and coordination. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Connect clients with partner agencies. • Help with applications and appointments for client records. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with client records. Working arrangement • Full-time, Monday to Friday. • On site in Hartford, with some flexibility after the first few months. • We will tell you what to expect at each step of the hiring process.
