Northwind Glen Care Services is looking for a Case Manager. The job centres on benefits navigation for care plans, and it would suit someone who likes steady, practical work. You would join a small organisation of about 30 people based in Hartford. What you will do • Follow up regularly and update records accurately. • Help with applications and appointments for care plans. • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with care plans.
