Case Manager - benefits navigation (Fresno, CA)

Westbrook Birch Care Services Group is a social-care provider serving older adults and people with disabilities around Fresno. We are hiring a Case Manager to take on benefits navigation. We train and promote from within. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Fresno. • We will tell you what to expect at each step of the hiring process.
