Case Manager - benefits navigation (Fresno, CA)

Clearpath Forge Care Services is a social-care provider serving older adults and people with disabilities around Fresno, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 165 people based in Fresno. What you will do • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Fresno, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
