Case Manager - benefits navigation (Des Moines, IA)

Lumen Fern Care Services is a social-care provider serving older adults and people with disabilities around Des Moines, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for family communication. • Meet clients, understand their needs and build a simple plan. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Contract for six months, with clear deliverables from the first week. • On site in Des Moines. • Applications are reviewed on a rolling basis.
