Case Manager - benefits navigation (Denver, CO)

Summit Yarrow Health runs clinics and patient services around Denver, and we need a Case Manager to lead our benefits navigation. You would join a large organisation of more than 1,000 people based in Denver. What you will do • Connect clients with partner agencies. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for insurance authorisations. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with insurance authorisations. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, with flexible days. • On site in Denver. • We will tell you what to expect at each step of the hiring process.
