Case Manager - benefits navigation (Dallas, TX)

Clearpath Health Group provides primary and specialty care in Dallas. We are hiring a Case Manager to take on benefits navigation. You would join a large organisation of about 633 people based in Dallas. What you will do • Help with applications and appointments for patient scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. Nice to have • Bilingual English and Spanish. • Experience with patient scheduling. Working arrangement • Part-time, about 28 hours a week, afternoons. • On site in Dallas. • Applications are reviewed on a rolling basis.
