Case Manager - benefits navigation (Dallas, TX)

We are growing our team at Clearpath Ashton Health Partners and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of about 715 people based in Dallas. What you will do • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for electronic records. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with electronic records. Working arrangement • Full-time, Monday to Friday. • On site in Dallas, with some flexibility after the first few months. • We reply to every applicant.
