Case Manager - benefits navigation (Columbus, OH)

We are growing our team at Cedar Oak Health Group and adding a Case Manager whose main responsibility will be benefits navigation. We are busy and kind, and we look after each other. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for patient education. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient education. • Bilingual English and Spanish. Working arrangement • Contract for nine months, with the possibility of becoming permanent. • On site in Columbus. • Applications are reviewed on a rolling basis.
