Case Manager - benefits navigation (Chicago, IL)

Silverline Forge Care Services is a social-care provider serving older adults and people with disabilities around Chicago, and we need a Case Manager to lead our benefits navigation. You would join a small organisation of about 34 people based in Chicago. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for family communication. What we are looking for • Experience in social services, casework or a related field. • Strong documentation habits. • Empathy, patience and good boundaries. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Contract for three months, with clear deliverables from the first week. • On site in Chicago. • We reply to every applicant.
