Highbridge Oak Health Partners is looking for a Case Manager. The job centres on benefits navigation for clinic supplies, and it would suit someone who likes steady, practical work. We are busy and kind, and we look after each other. What you will do • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Help with applications and appointments for clinic supplies. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with clinic supplies. • Bilingual English and Spanish. Working arrangement • Part-time, about 20 hours a week, mornings. • On site in Charlotte. • Applications are reviewed on a rolling basis.
