Case Manager - benefits navigation (Charlotte, NC)

Lumen Fern Care Services Group provides in-home and community support in Charlotte, and we need a Case Manager to lead our benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for visit scheduling. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Full-time, on a regular day shift. • On site in Charlotte, with some flexibility after the first few months. • Applications are reviewed on a rolling basis.
