Case Manager - benefits navigation (Charleston, SC)

We are growing our team at Ironwood Basin Care Services Group and adding a Case Manager whose main responsibility will be benefits navigation. The work is personal and so is the culture. What you will do • Meet clients, understand their needs and build a simple plan. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with visit scheduling. Working arrangement • Contract for nine months, with clear deliverables from the first week. • On site in Charleston. • We reply to every applicant.
