Case Manager - benefits navigation (Buffalo, NY)

We are growing our team at Harborlight Orchard Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would join a large organisation of about 288 people based in Buffalo. What you will do • Report outcomes to funders in clear, short summaries. • Help with applications and appointments for caregiver training. • Follow up regularly and update records accurately. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Contract for four months, with the possibility of becoming permanent. • On site in Buffalo, with some flexibility after the first few months. • We reply to every applicant.
