Foxglove Ivory Health Group is looking for a Case Manager. The job centres on benefits navigation for patient education, and it would suit someone who likes steady, practical work. You would join a large organisation of about 208 people based in Buffalo. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. • Connect clients with partner agencies. • Help with applications and appointments for patient education. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with patient education.
