Case Manager - benefits navigation (Buffalo, NY)

Greenridge Fern Care Services provides in-home and community support in Buffalo. We are hiring a Case Manager to take on benefits navigation. You would join a small organisation of about 18 people based in Buffalo. What you will do • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. • Help with applications and appointments for caregiver training. • Report outcomes to funders in clear, short summaries. What we are looking for • Empathy, patience and good boundaries. • Strong documentation habits. • Experience in social services, casework or a related field. Nice to have • Experience with caregiver training. • Bilingual English and Spanish. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Buffalo. • We will tell you what to expect at each step of the hiring process.
