Case Manager - benefits navigation (Boston, MA)

We are growing our team at Greenridge Fern Care Services and adding a Case Manager whose main responsibility will be benefits navigation. You would join a small organisation of about 25 people based in Boston. What you will do • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Help with applications and appointments for visit scheduling. • Follow up regularly and update records accurately. • Report outcomes to funders in clear, short summaries. What we are looking for • Strong documentation habits. • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. Nice to have • Experience with visit scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Boston. • We will tell you what to expect at each step of the hiring process.
