Case Manager - benefits navigation (Birmingham, AL)

We are growing our team at Stonegate Drift Health and adding a Case Manager whose main responsibility will be benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for patient scheduling. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Connect clients with partner agencies. • Follow up regularly and update records accurately. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Experience with patient scheduling. • Bilingual English and Spanish. Working arrangement • Full-time, Monday to Friday. • On site in Birmingham. • Applications are reviewed on a rolling basis.
