Case Manager - benefits navigation (Baltimore, MD)

Summit Keystone Care Services supports families and individuals in Baltimore with personal care and coordination, and we need a Case Manager to lead our benefits navigation. The work is personal and so is the culture. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Experience in social services, casework or a related field. • Empathy, patience and good boundaries. • Strong documentation habits. Nice to have • Experience with family communication. • Bilingual English and Spanish. Working arrangement • Part-time, about 24 hours a week, with flexible days. • On site in Baltimore. • We will tell you what to expect at each step of the hiring process.
