Case Manager - benefits navigation (Anchorage, AK)

Fernwood Harbor Care Services Group is a social-care provider serving older adults and people with disabilities around Anchorage. We are hiring a Case Manager to take on benefits navigation. You would work closely with colleagues in operations, support and management. What you will do • Help with applications and appointments for family communication. • Report outcomes to funders in clear, short summaries. • Meet clients, understand their needs and build a simple plan. • Follow up regularly and update records accurately. • Connect clients with partner agencies. What we are looking for • Empathy, patience and good boundaries. • Experience in social services, casework or a related field. • Strong documentation habits. Nice to have • Bilingual English and Spanish. • Experience with family communication. Working arrangement • Part-time, about 28 hours a week, mornings. • On site in Anchorage. • We will tell you what to expect at each step of the hiring process.
