When does my employer coverage actually end? - what worked for you?

I am not sure what date my benefits stop after being let go as a quality inspector. I have read a lot of threads here and could not find my exact situation. I do not want to be surprised by a bill. What should I do about appointments?

Thanks for writing this out. I went through something close to it last year, and it settled down once I kept a simple log of every call and form.
