When does my employer coverage actually end? (update)

I am not sure what date my benefits stop after being let go as a production planner. I am in Birmingham, in case local advice matters. The letter is unclear about dental and vision. Did coverage match what HR said?

I moved my appointments earlier while coverage was active. It saved me a bill.
