When does my employer coverage actually end? (Tulsa)

HR said my health plan continues for a bit and I do not understand how. I have been trying to stay organised about it. I do not want to be surprised by a bill. Did coverage match what HR said?

Dental and vision sometimes end on a different date from medical. Mine did.
