When does my employer coverage actually end? - first time posting

HR said my health plan continues for a bit and I do not understand how. I have been trying to stay organised about it. The letter is unclear about dental and vision. What should I do about appointments?

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
