When does my employer coverage actually end? - any advice?

I am not sure what date my benefits stop after being let go as a mechanic. I do not usually post, so bear with me. The letter is unclear about dental and vision. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
