When does my employer coverage actually end? after 7 years as a payroll clerk

I am not sure what date my benefits stop after being let go as a payroll clerk. Any experience at all would help. The letter is unclear about dental and vision. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
