When does my employer coverage actually end? after 6 years as a warehouse supervisor

HR said my health plan continues for a bit and I do not understand how. This is my first time dealing with any of this. The letter is unclear about dental and vision.

Keep everything in writing and save the confirmation numbers. It saved me later.

Dental and vision sometimes end on a different date from medical. Mine did.
