When does my employer coverage actually end? after 4 years as a store manager

HR said my health plan continues for a bit and I do not understand how. It has been a long 23 weeks. I have an appointment next week. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.

Keep everything in writing and save the confirmation numbers. It made the follow-up calls so much easier.
