When does my employer coverage actually end? after 4 years as a machine operator

I am not sure what date my benefits stop after being let go as a machine operator. It has been weighing on me all week. I have an appointment next week. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
