When does my employer coverage actually end? after 3 years as an IT help-desk technician

I am not sure what date my benefits stop after being let go as an IT help-desk technician. I have read a lot of threads here and could not find my exact situation. I have an appointment next week. How did you confirm your end date?

Dental and vision sometimes end on a different date from medical. Mine did.
