When does my employer coverage actually end? after 16 years as a software tester

HR said my health plan continues for a bit and I do not understand how. I do not usually post, so bear with me. The letter is unclear about dental and vision. How did you confirm your end date?

Dental and vision sometimes end on a different date from medical. Mine did.
