When does my employer coverage actually end? after 15 years as a line cook

I am not sure what date my benefits stop after being let go as a line cook. I do not usually post, so bear with me. I have an appointment next week. What should I do about appointments?

Dental and vision sometimes end on a different date from medical. Mine did.
