When does my employer coverage actually end? after 11 years as a school bus driver

HR said my health plan continues for a bit and I do not understand how. I do not usually post, so bear with me. I have an appointment next week. What should I do about appointments?

I moved my appointments earlier while coverage was active. It saved me a bill.
