When does my employer coverage actually end? after 10 years as a logistics coordinator

I am not sure what date my benefits stop after being let go as a logistics coordinator. I have read a lot of threads here and could not find my exact situation. The letter is unclear about dental and vision. Did coverage match what HR said?

Dental and vision sometimes end on a different date from medical. Mine did.
